Advance Care Planning

$120/session +HST (available in-person or virtually)

Planning for your End-of-Life Journey is an essential step not only for yourself but also for your loved ones. By taking the time to identify and document your wishes, you can ease decision-making during a time that can be emotionally charged and overwhelming.

1. Identify Your Substitute Decision Maker: Choose a trusted individual who can advocate for your wishes when you are unable to do so. This person should be someone who understands your values and preferences regarding medical treatment and end-of-life care.

2. Establish Powers of Attorney: Designate a durable power of attorney for healthcare decisions. This allows your chosen representative to make medical decisions on your behalf if you become incapacitated. Additionally, consider appointing a financial power of attorney to manage your financial matters, ensuring that your bills are paid and assets are properly managed in accordance with your wishes.

3. Articulate Your Post-Mortem Wishes: Articulate your preferences regarding post-mortem arrangements, including burial or cremation, memorial services, and organ donation. Document these wishes clearly to provide guidance for your family and alleviate uncertainty at a challenging time.

4. Regularly Review and Update Your Plans: Life circumstances change, and so may your preferences. Regularly reviewing and updating your documents ensures that they remain aligned with your current values and wishes.

By proactively addressing these aspects of your End-of-Life Journey, you can provide clarity and support to your loved ones, allowing them to focus on honouring your legacy during a time of grief.

Companion Care

$40/hour +HST (in-person only, maximum 3 hours/day)

This personal connection can significantly enhance the quality of life for aging seniors, or those with life-limiting illnesses by providing not only companionship, but also assistance with daily activities.

Companion care involves understanding the unique emotional and physical challenges faced by individuals. I offer a listening ear, engage in meaningful conversations, and create a sense of normalcy and joy in daily routines. Whether it’s sharing a hobby, going for a walk, or simply reminiscing about the past, the bond formed during these interactions can alleviate feelings of isolation and loneliness.

Moreover, companion care goes beyond social interaction. It can also include help with light housekeeping, medication reminders, or legacy projects, ensuring that people maintain their independence while receiving the support they need. As we recognize the vital importance of connection, investing in companion care can provide individuals with a fulfilling and enriching experience, mitigating the emotional toll of aging and/or the dying experience.

Companion care can also include respite for family or friends during an active vigil for their dying loved one by providing continuous support and a physical presence.
